Page numbers you type always refer to the document's original layout, so "insert after 3,6,9" means the 3rd, 6th, and 9th pages of the file you uploaded.

Yes — the inserted pages are genuinely empty, with zero content of any kind (no text, headers, footers, or graphics). If your original document has headers/footers on every other page, those will NOT automatically carry over to the blank pages — you'd need to add them separately (e.g. with the PDF Header & Footer tool) if you want the blanks to match.
